Subjective Data
Chief Complaint (CC): “I can’t remember things”
History of Present Illness (HPI): 85-year-old Caucasian female admitted to this facility under a
Baker Act while being evicted from her home. She does not understand why she is being evicted
and does not know what month it is or who the president is. It was reported that she stopped
taking her medications, has no running water in her home, and is unable to care for herself.
Medications: Patient denies any medications at home. Medications ordered while in this facility
are: Tylenol 650 mg PO Q6 PRN, Zofran 4 mg PO Q6 PRN, Nicotine patch 21 mg TD on in the
AM off in the PM, Melatonin 9 mg PO QHS, Duoneb INH Q6 x 5 days, Tessalon pearls 100 mg
PO TID x 5 days, Mucinex 1 tablet PO TID x 5 days, Zithromax 500 mg PO x 1, then 250 mg
PO daily x 4 days
The Beer’s criteria tool was developed to improve medication safety among the elderly.
After reviewing this patient’s current medications and comparing them with the Beers Criteria, I
have discovered that the medications this patient is taking are appropriate for her age.
(American Geriatrics Society, 2015).
Allergies: Penicillin
Past Medical History (PMH): Patient reports asthma, chart says COPD
Past Surgical History (PSH): Patient denies
Personal/Social History: Patient reports she quit drinking and smoking 2 years ago, her chart
says she smokes 1 ppd
