What does one of the five symptoms sympto ... ms HAVE to be to categorize as a MDE?: depressed mood or loss of interest or pleasure -Can have all of the symptoms but if you do not have a loss of interest or a depressed mood then it is not a MDE, must have one of these at least but could have both and 3 other symptoms 3. To qualify as a MDE, the symptoms have to be from previous function.: a change example: May have a Low affect for their personality - but is there a change from before? Baseline is so important! 4. Symptoms of MDE: -Depressed mood most of the day, nearly every day -Markedly diminished interest or pleasure in all, or almost all, activities most of the day nearly every day •Significant weight loss when not dieting or weight gain or decrease or increase in appetite nearly every day •Insomnia or hypersomnia nearly every day •Psychomotor agitation or retardation nearly every day •Fatigue or loss of energy nearly every day •Feelings of worthlessness or excessive or inappropriate guilt nearly every day •Diminished ability to think or concentrate, or indecisiveness nearly every day •Recurrent thoughts of death, recurrent suicidal ideation without a specific plan, or a suicide attempt or a specific plan for committing suicide 5. Second criteria for MDE: The symptoms cause clinically significant distress or impairment in social, occupational, or other important areas of functioning 6. Episodes must be separated by at least for it to be a major depressive episode.: 2 months 7. What is major depressive disorder?: never been without the symptoms in Criteria A and B of MDE for more than 2 months at a time -composed of episodes 8. If the episode is attributed to drugs, is it still an MDE?: The episode must not attributable to the physiological effects of a substance or another medical condition -Have to make sure that there is not something physiological happening to deter- mine the diagnosis As an OT this is important because this impacts how we do treatment, we may need to focus on med management or something if this is the case 9. How long does mood disturbance need to sustain to be considered mania?: Lasting at least 1 week and present most of the day, nearly every day -nearly everyday= more days than not 14. Is goal directed behavior efficient all the time?: Just because something is goal directed that does not mean it is effective or efficient 15. To be mania, the person must meet at least 3 of the following criteria during the period of mood disturbance: (criteria B): During the period of mood disturbance, increased energy/activity • Inflated self-esteem/grandiosity • Decreased need for sleep •More talkative than usual •Flight of ideas, racing thoughts • Distractibility • Increase in goal-directed activity; psychomotor agitation • Excessive involvement in activities that have a high potential for painful conse- quences 16.
